The Sell Back Request form must be filled out in its entirety and signed in front of a Notary Public. Do not sign until you are in the presence of the Notary. You must return any lot deeds that you have at the time the request is submitted.

You will be reimbursed in the amount that was paid for the grave space at the time that it was originally purchased, not the current value.

Grave lots can’t be sold to another person. They may only be sold back to the township.
